Please fill out this form in order to be considered to coach for the 2023 Cheer Season. Coaches will go through an interview process and experience coaching is not necessary.

Head Coaches must be 21 years or older
Assistant Coaches must be 14 years or older.

All coaches over the age of 18 will be required to complete a National Association for Youth Sports certification, a background check and must attend a coaches clinic/meeting. You will be notified via email if you are chosen as a coach.

All players are required to submit proof of grade. 

Starting this 2023 Tackle Football season, CLYFL will accept the following forms for proof of grade:

 

• Picture of your child's current student ID for the current academic year.
• Picture of your child's student portal with the current grade for the current academic year.

Please submit your proof to Amy Hartman, Director of Registration, at registration@clyfl.org. You may also show it to a board member during the first week of practice.

 

Physical Examination Form

All players are required to have a physical examination form on file. The form is available for download on our website under “documents” and “athlete forms”. Once you have the form filled out and signed, please send it to Melissa Martini, Director of Safety, at safety@clyfl.org.  You may also bring your completed form to equipment pick up. 

We do require all physical forms to be completed before the first practice.
